The Community Outreach Specialist is responsible for identifying Medicaid-eligible patients in need of home care services and assisting eligible patients with the Medicaid enrollment process. This role focuses on community engagement, education, and partnership-building with local organizations, healthcare providers, and community leaders to increase awareness of our agency’s services and help underserved individuals access the support they need. The specialist will work closely with potential patients and their families, ensuring they understand their options and guiding them through Medicaid enrollment and eligibility steps. This position requires a strong understanding of Medicaid regulations, the LHCSA landscape, and local community resources.
