In this article, we’ll walk you through the process of nursing and rehabilitation in Brooklyn. We’ll effectively cover admission to discharge in this article. Without further ado, let’s dive in.

Admission to the Nursing and Rehabilitation Center

The first step in the nursing and rehabilitation process in Brooklyn is the admission process. There are many ways that admission can occur. This could be through referrals from physicians or even family members. After admission, the patient would be assessed in the nursing center to determine the level of care they require.

Assessment at the nursing center includes medical history, physical abilities, and evaluation of mental health concerns. The assessment will determine the care plan that will be developed for the patient. We always take the patient’s rehabilitation needs and goals into consideration when creating a care plan at the Brooklyn Center.

The Nursing Process

This is a crucial part of the entire process when you enter the nursing home. After admission, the nursing process kicks off. Older adults get round-the-clock care because of their fragile nature. Our healthcare professionals will work hard to provide comprehensive care for those recovering from an injury or surgery. The nursing stage usually involves four crucial steps: assessment, planning, implementation, and evaluation.

Assessment

The healthcare team focuses on gathering crucial data during the assessment phase concerning the patient’s medical history, present state, and rehabilitation objectives. Our top-notch medical professionals will give you an accurate assessment of your physical health, mental health, and mental function.

During the assessment stage, the patient’s background details are also obtained. In addition, our professional team will gather information about the patient’s previous hospitalization and their response to pain and pain relief measures. Sleep patterns are also crucial components of a comprehensive assessment. To also effectively address the patient’s emotional needs in the patient-centered care environment, their coping strategies and self-esteem will be considered.

To provide skilled nursing care, continuous physical assessments will be conducted throughout your rehabilitation process. Additionally, the patient’s participation in care and ability to demonstrate self-care in areas like wound cleaning, eating, and others would also be documented. Notably, priorities would vary at various points during the rehabilitation stage.

Planning

Based on your assessment, a care plan would be carefully created outlining the patient’s needs and goals for rehabilitation. The care plan is carefully developed and individualized to meet the unique needs of the patient. Sometimes, it includes physical therapy, occupational therapy, and many more.

During this phase, our skilled nursing team considers several factors, like the patient’s medical history, physical abilities, cognitive function, and mental health status. This care plan is usually created in collaboration with the patient and their family. Additionally, the plan will be continually updated as the patient’s goals and needs change.

Implementation

The implementation phase usually involves the healthcare team providing care according to the outlined plan. This usually involves providing therapy, medication, and other treatments prescribed in the care plan. Our customized nursing home care solutions also involve closely monitoring the patient’s progress and providing ongoing support and education. Additionally, the team would also consult with physicians, therapists, pharmacists, and other healthcare professionals to ensure patients received the best short- and long-term care.

Evaluation

During the implementation of the care plan in our downtown Brooklyn nursing center, the skilled nurses and other professionals would consistently evaluate the patient’s progress while adjusting the care plan based on the patient’s needs. Most of the time, the healthcare team would evaluate patients’ physical and mental health status using cutting-edge technologies. Based on the results, the patient’s care plan would be modified.

The healthcare team uses different tools to ensure the patient’s progress during the evaluation phase. Some of these tools could include physical, functional, and cognitive assessments. Additionally, the team would consider the patient’s feedback and input to determine the effectiveness of the innovative care plan.

Overall, the nursing process’s assessment, planning, implementation, and evaluation phases are essential components integrated into our healthcare solutions. These phases ensure that patients receive customized and innovative care in our boutique-style living environment. These phases also ensure that the care plan meets the patient’s unique needs and goals for rehabilitation. By working closely with the patient and their family, our healthcare professionals can provide the best care and support for a successful recovery from injuries and surgeries.

The Rehabilitation Process

The nursing care process must include the rehabilitation phase. It is a crucial step in the recovery process following an operation, illness, or injury. Patients can regain function and enhance their general quality of life by going to the Brooklyn rehabilitation facility. Here are some of the main arguments in favor of the necessity of rehabilitation.

It Restores Physical Function

Physical injuries or impairments such as joint stiffness, muscle weakness, or balance problems can be addressed with rehabilitation to help patients get better. Physical therapy and other interventions can help patients get back their strength, functionality, and mobility. Patients who regain physical function can resume everyday activities and significantly lessen their dependence on others for care.

Improving Mental Health

Rehabilitation can also significantly enhance a patient’s mental health. A patient with physical injuries or illnesses might experience depression, anxiety, or mental health issues. In Brooklyn, NY, medical professionals can utilize various forms of therapy to significantly lessen patients’ stress, enhance mood, and improve overall well-being.

Promoting Independence

Patients who undergo rehabilitation can regain their independence and self-esteem. Patients have a higher chance of feeling confident and deserving of themselves if they can handle daily tasks like cooking, dressing, and grooming on their own. This can also encourage patients to participate more actively in their daily lives.

It Prevents Future Injuries

Rehabilitation can also help prevent future injuries in patients as well as complications. Physical therapy and different interventions help patients effectively enhance their strength and flexibility, which would reduce the risk of falls and other types of injuries. Furthermore, the skilled nursing team can teach patients how to move properly and safely, preventing complications and injuries in the future.

Improving Quality of Life

Rehab can also enhance patients’ overall quality of life by helping them achieve their recovery-set goals. By tailoring care plans to a patient’s pre-injury or illness level of function, long or short-term rehabilitation can assist patients in enjoying and living a more fulfilling life.

Overall, the rehabilitation process is vital for patients recovering from illnesses, injuries, or surgeries. The rehab process is a crucial component of the nursing and rehabilitation process in Brooklyn, and healthcare professionals work closely with patients to provide individualized care.

Improving Body Image and Self-Concept

One of the phases of the rehabilitation process is improving body image. Patients who suffer from injuries or illnesses tend to also suffer from a loss of body image. As care progresses, patients recovering from injuries and surgeries experience basic concerns like how long they’ll remain in rehab, what happens to their job, and how they’ll pay for the services rendered.

As patients express their concerns, the nurses listen and provide realistic support as part of the rehabilitation process. A major responsibility of the skilled nursing team is constantly assessing the patient’s psychosocial reaction. Nurses need to discover the patient’s concerns and fears. Awareness of these anxieties and understanding the basis of the patient’s fear will help the nurse provide support and develop a plan to help the patient deal with these feelings.

Forms of Therapy

The rehabilitative phase usually involves different forms of therapy. Some of them include the following:

Physical Therapy

This therapy form focuses on the physical function of the patient. The physical therapist closely works with the patient to improve their balance, strength, mobility, and flexibility. Physical therapy can include varying forms of stretches, exercises, and other innovative techniques. Therapists might also use assistive devices like walkers or crutches to help patients move around safely.

Occupational Therapy

This is another type of therapy used during the rehabilitation process. It focuses on improving the patient’s ability to perform various activities. The therapist works with the patient to significantly improve their ability to perform various tasks. Occupational therapy sometimes involves activities and exercises that improve the patient’s coordination and motor skills.

Speech therapy

This form of therapy focuses on improving the patient’s ability to communicate effectively. The speech therapist works with patients to improve their language, speech, and cognitive abilities. Speech therapy could also improve speech and cognitive abilities. Speech therapy could also include unique techniques to help patients speak more clearly and remember information.

Each therapy session is tailored to the needs and goals of each patient. With the combination of therapies and techniques, healthcare professionals can help patients regain function and independence while fostering successful recovery.

Discharge Planning

As the recovery phase gets shorter during rehabilitation, the focus shifts toward outpatient care. Efforts are made to prepare the patient and family for the care they’ll have to continue at home. The skilled nursing team starts to instruct patients about the measures and procedures they’ll have to perform. In addition to providing wound care, to patients and their families, the nursing team would also provide patients and families with written and verbal instructions about preventing complications, nutrition, and pain management.

Patients are also taught to recognize abnormal signs and immediately report them to the world-class physicians available. This information will help patients progress successfully through the rehabilitation phase. Toward the end of the rehabilitation phase, the nursing team assists the patient and their loved one in planning for the patient’s continued care by identifying and acquiring equipment and supplies needed at home.

Continuing Care

Follow-up care is also necessary, and preparation needs to begin at the early stages of care. Patients might need to return to the center periodically for evaluation and medication and for home care instructions. Some patients who return home after rehabilitation require various forms of therapy. It is often the nurses who are responsible for coordinating all aspects of care during this period.

Some patients require the assistance of a home health nurse or social worker. The home care nurse monitors the progress of the patient and their adherence to the care plan. They also note any problems interfering with the patient’s ability to receive care.
