Social model day care facilities are not permitted to offer any medical care, including something as easy as helping you take your medication. At Four Seasons, we are a medical model facility which means we have a full-time medical staff, therapists, nutritionists, and social workers to make sure you’re receiving the care you need, properly. And we do it all in a very social atmosphere, including daily activities, entertainment, seasonal outings and so much more.
Absolutely. A fleet of custom vans picks up members at their door every day and returns them home at the end of each session. And of course, we provide all transportation necessary for members who are participating in various outings and trips.
We offer services in two daily sessions (morning and afternoon) and we’re open six days a week, holidays included. Some members attend every day, while others come only once or twice a week. We’ll create a membership schedule that best serves your special needs.
Yes, it is, but even if you don’t have Medicaid, or were told previously that you were not eligible, we may still be able to qualify you for Adult Day Health Care services under Medicaid. Call us first to find out exactly what you qualify for, then make the choice that’s best for you.
Yes, you can. Many of our members attend one or two days a week and still receive care in their own homes once or twice a week. The best way to determine the right combination for your situation is to just call us. We’ll help you determine everything you’re entitle to and see that you get into right program.
